MY JOB ROLE IS TO SUPPORT AND ASSIST VUNERABLE PEOPLE IN THE COMMUNITY TO BECOME INDEPENDENT AFTER A ILLNESS OR DISCHARGE FROM HOSPITAL.AND TO BE ABLE TO LIVE AT HOME UNTIL IT IS OTHERWISE INPRACTICAL TO DO SO. I GAINED MY NVQ LEVEL 2 IN HEALTH AND SOCIAL CARE AND I ALSO GAINED MY PALLATIVE CARE LEVEL 3 A FEW YEARS AGO.
