With 34 years of professional experience, delivering frontline care and support I facilitate mentoring, advocation and training for individual educational and skill development.
Ive provided health and social care support for impaired or disadvantaged people of all ages with dual, learning, emotional and physical disabilities. I also supported people with autism, epilepsy, mental health, dementia and who present challenges.
Im hard working and vocationally committed with a competent skill set and enthusiastic attitude. I also provided outreach support, administer and audit medication, and end of life, respite care within community, residential, hospital, resettlement, charity and social facilities.
With a strong field based target focus, I coordinate ‘individually philosophised” package support, with reviews to develop and implement independent goal plans.
Utilising achievement indicators I also adapt quickly to company and legislative philosophies to ensure health and safety and quality service delivery.
Holding key responsibilities during the managers absence, included shift and staff coordination, security, auditing, roster facilitation, medication and administration and risk assessment. My administrative responsibilities include following financial, security and data protection procedures.
Im an articulate communicator with reception experience including liaising with disciplinary teams, clients, family members, and community members and I proactively react to urgent emergency situations and task management. Understanding the importance of health, safety and best practice, I non-judgmentally respect difference, equality and diversity. Ive also undergone extensive training to develop transferable skills, including Level 2 Health and Social Care Diploma for which I hold experience and competencies.
Managing caseloads I proactive support disabled or disadvantaged rent payers including homeless, substance dependants or mental health survivors to achieve independence
Enabling choice and access to sheltered, council and private accommodation involved liaising with health, social, welfare or voluntary services to build links and promote client involved support
I design, develop assess and identify required program plans to maximise income, benefits, education and environment management skills to maximise independent home care choices.
I monitor referral criteria and review assessments, appeals and occupancy agreements,
I encourage customer relations and address neighbour disputes and professional challenges with sensitive effectiveness. I attended surgeries to identify breach concerns, risk defects to resolve complaints or advise solutions to action emergency responses. I also prepared rooms for occupants and coordinated furnishing removals and arranged transport with continued care options.
I can access case law and policies to regulate standards or alert whistle blowing safeguards in line with procedure.I attended training and supervisions or court hearings, I also provided outreach, home visits and inspections and I also represent services during organisation meetings and agency reviews.
