I have experience working in private homes with direct families. I am entrusted with the great responsibility of safeguarding my service users (the majority are vulnerable adults) in their homes. I work with a variety of patients with both physical and mental health problems, which requires a holistic approach to ensure that their health and well-being needs are met.
I deliver individualised care plans for patients with mental health conditions such as dementia, psychosis, schizophrenia, learning difficulties, and autism based on their “This is me” documentation. This enables me to maintain patients' respect, rights, and their dignity. The transferable skills can be applied to the clinical care environment. Part of my job experience includes escorting patients and service users to appointments, outdoor activities, and errands as required.
