Person -centred thinking tools are essential for effective communication among members of the multidisciplinary teams to ensure that there is equal and consistent treatment of all patients. I also have extensive knowledge of caring for people with learning disabilities and mental health problems. Similarly, I also have experience in writing care plans, documenting information, delivering handovers and working alongside multidisciplinary teams. I believe in delivering a high standard of support and always strive to enable clients to reach their full potential and offer opportunities to enable them to lead an independent life. Similarly, I support service users to access community events, attend medical appointments and I strongly believe in the Person-Centred care concept.
