A highly motivated Social Worker with significant and progressive experience in developing person-centred care plans and excellent assessment for vulnerable adults with physical and mental impairment in hospital discharge team, wellbeing community team, voluntary sector and drug and alcohol service and adults with learning. My Experience as a social worker, social work assistant, on placement and as a support worker/health care assistant/ Live-in-carer has developed my skills, knowledge, and confidence in writing excellent assessments and person-cantered care plans that represents each individual service user’s needs. My experience also improved my ability to advocate for service users during crisis and worked in partnership with service users and their family, colleagues, other agency to achieve service user’s desired outcome.
Having worked as a social worker for 5 years plus, I am looking at going back to providing one-to-one care.
