My experience has included providing care for three clients (different times) diagnosed with advanced Parkinson's disease with comorbidities (such as diabetes). In the cases I administered prescribed medications every three to four hours and offered PRN medication according to symptoms, following guidelines set by their GP while adhering to dosage limitations. I also patiently assisted clients patiently during episodes of tremors or confusion and prepared appropriate food considering their swallowing abilities. In addition I offered reassurance and support to clients experiencing hallucinations and ensured the safe care of any medical pumps that had been put on the patient by the district nurses.
I am also knowledgeable in catheter care and I have taken care of clients using various catheters including condom catheters, indwelling catheters, suprapubic catheters and nephrostomy bags. I empty and replace the bags as required and attach night bags as determined by the healthcare team. I also know how to look out for changes in the urine colour or texture or quantity that may indicate an infection, dehydration or blockage of the catheters and immediately escalate as per procedures or careplan.
