New hope care has been providing supported living to adults diagnosed with a learning disability, autism, mental health, behaviours that challenge and associated health conditions – diabetes, epilepsy, since 2007. We support s/users in their own tenancies in various projects, we assist with securing the accommodation by working closely with landlords and ensuring the accommodation is acceptable and safe and in a location of their preference. We assist with welfare benefits claims and with all general day to day administration tasks. We provide support to s/users that have moderate to complex care needs, providing 1:1 support, 2:1 support and shared hours support.
We work with individuals in a person centred manner, meaning we engage fully and involve all s/users within their daily and life decision making skills; we support with capacity issues and involve key professionals when required. We encourage and aim to motivate s/users with using community facilities and be a presence within their own communities.
We encourage safe risk taking and have clear risk assessment plans for everyday risks that are due to cognitive actions.
Our documentation is in written word and pictorial formats.
We implement communication passports; health action plans and aim for s/users to have regular GP checkups on an annual basis.
We assist with financial matters, whether a s/user has a personal budget/direct payment our accounts dept. ensure there is a clear financial management plan and audit process in place to keep monies safe.
We have an experienced Management team, ranging from social worker, nurses, RMA awards, NVQ4/5 etc.
We document the s/users journey with the service and highlight key milestones to provide evidence that outcomes are being realised when achieved. We promote independence skills and work closely with Professionals to achieve the best possible ways in how we can support this process, either through staff training or better use of technology.
We implement REACH reviews to ensure we are looking at best possible ways of achieving quality of life outcomes.
